Assessment Fees

We require a down payment of $2,500 for evaluations with any remaining balance due at the feedback session (when testing results and recommendations are discussed and is usually done the same day of testing.) Please note that testing costs vary and may be more or less depending on the scope of the evaluation, presenting concerns, time spent reviewing records, and the age of you or your child. A "superbill" will be provided after the feedback session, which will include all appropriate diagnostic and billing codes for direct submission by the patient’s family to their insurance company. Please note that many insurance companies require pre-authorization prior to testing. If this is the case, you will need to provide a hard copy of the pre-authorization form with the submission fax number prior to your appointment. The pre-authorization process usually takes approximately one week in order to grant or deny coverage. This process is between you and your insurance company. Forest Psychological Clinic and its psychologists do not participate in appeal of decisions or any other negotiations with insurance companies. Testing is capped at $3,300.00 (even if your clinician spends more time on the evaluation.) In circumstances of unusual financial hardship, we may be willing to negotiate a fee adjustment and/or payment installment plan. 

Assessment or testing sessions are billed at a base rate of $260 per hour, billed in 30-minute increments with additional hours billed outside of session for scoring, interpretation, diagnostic formulation, report writing and feedback. Evaluations require psychologists to spend significant time both inside and outside of sessions administering tests, scoring, interpreting, and writing up results in a comprehensive report. The feedback session is scheduled, in most circumstances the same day, to explain the evaluation results to families. A comprehensive report is delivered to the family within one week of providing feedback. 

At Forest we offer a reduced rate for those seeking only an ADHD evaluation and are comfortable having a psychology graduate student administering the testing. A licensed psychologist will be in the exam room during testing and supervising the administration. For these evaluations there is a flat rate of $1000. In circumstances of unusual financial hardship, we may be willing to negotiate a fee adjustment and/or payment installment plan.
