At Forest Psychological Clinic, we believe in transparent pricing and clear policies to help you navigate the costs associated with our services. Below, you’ll find details on therapy fees, assessment fees, payment options, insurance coverage, and our refund policy.

Therapy Fees

Psychotherapy

Our psychotherapy fees vary depending on the clinician and session length. Fees are charged per therapy hour, and payment is expected at the time of service unless alternative arrangements are made.💡 In cases of financial hardship, we may consider fee adjustments or a payment installment plan. Please discuss this with your clinician if needed.

Phone Calls

Brief phone calls (5 minutes or less) related to scheduling or minor concerns are free of charge.Longer phone calls are prorated at a rate of $100 for 30 minutes.

Cancellation & No-Show Policy

24-hour notice is required for cancellations to avoid a fee. Cancellations made within 24 hours of your scheduled appointment will result in a cancellation fee equivalent to your psychotherapy session rate, which will be charged at your next appointment.

Insurance for Therapy Services

We currently accept the following insurance plans for therapy:

- Lyra

- Kaiser

- Regence - BlueCross BlueShield of Oregon

- Pacific Source

- Providence Health Plan

- ComPsych* For insurance plans marked with *, a referral from your primary care physician may be required. Please check with your insurance provider for coverage details.

Assessment Fees

Out-of-Pocket Payment

A down payment of $2,500 is required for psychological evaluations.Any remaining balance is due at the feedback session, where test results and recommendations will be discussed. Total costs may vary depending on the scope of the evaluation, presenting concerns, time spent reviewing records, and the patient’s age.

💡 Testing fees are capped at $3,500, even if the clinician spends additional time beyond the cap.

Assessment & Testing Sessions

Billed at a base rate of $260 per hour in 30-minute increments.Fees include time spent administering tests, scoring, interpreting results, diagnosing, report writing, and feedback discussions.A comprehensive report is provided within one week after the feedback session.

Lower-Cost ADHD Evaluations

We offer a reduced-rate ADHD evaluation for $1,000 when conducted by a psychology graduate student under licensed psychologist supervision.The supervising psychologist will be present in the exam room during testing.Payment plans may be available in cases of financial hardship.

Insurance for Assessments

We currently accept the following insurance plans for assessments:

For insurance plans marked with *, a referral from your primary care physician may be required. Please check with your insurance provider regarding pre-authorization requirements.

Insurance Reimbursement & Pre-Authorization

A superbill will be provided after the feedback session for direct insurance reimbursement.Some insurance companies require pre-authorization before testing. If required, you must provide a hard copy of the pre-authorization form with the submission fax number before your appointment.The pre-authorization process typically takes one week to approve or deny coverage.

Please note that Forest Psychological Clinic does not participate in appeals or negotiations with insurance companies regarding coverage decisions.

Refund Policy

Therapy Sessions

Payments for completed therapy sessions are non-refundable. If a session is prepaid but canceled with at least 24 hours' notice, a full refund or session credit will be issued.If a session is canceled with less than 24 hours’ notice, no refund will be provided, and the cancellation fee will apply.

Psychological Assessments

The initial down payment ($2,500) is non-refundable once testing has begun. If testing is canceled before any evaluation begins, a partial refund may be considered, minus administrative fees. If an assessment is partially completed but not finalized, the remaining balance may be adjusted based on time spent on testing and report writing. Refunds are not issued for completed assessments, regardless of insurance reimbursement decisions.

Prepaid Packages & Deposits

Any prepaid therapy or assessment package is non-refundable once sessions have begun. Unused prepaid sessions may be credited toward future services or transferred to a family member upon request.

Insurance & Reimbursement

If your insurance claim is denied, you are still responsible for the full payment of services.Forest Psychological Clinic is not responsible for insurance disputes, reimbursement delays, or denied claims.
